The Perinatal Program at Trinity Home Health Services addresses the needs of the woman experiencing a high-risk pregnancy. The program provides obstetrical home care services to the pregnant woman in an effort to attain an optimal pregnancy outcome.
Trinity HHS works closely with and under the direction of an Obstetrician to reach optimum outcome for both the mother and child through a team approach. The team consists of the Physician, Nurse and Case Manager from the Health Plan.
The perinatal monitor center nurse works under the direction of the attending physician in carrying out individualized physician orders and collaborates with the patients physician as needed for successful management of the patient.

Continuum of Care
The monitor center is staffed with qualified perinatal registered nurses with a minimum of five years experience in labor and delivery.
Our establishment works closely with Case Managers, thus eliminating the need for their daily contact with the high-risk patient. Our weekly reports and 24/7 access to our web-based software also reduces the number of contacts necessary per patient per case manager.
